Why use topical or Transdermal medications for dental
problems?
Topical or Transdermal {trans = through; dermal = the skin} medications
are compounded to enhance the delivery of medication to the area that requires special
attention. These can include gels, ointments, creams, lotions, and lip balms.
These formulations rapidly enter the blood stream and have a low side effect
profile. Some of the topical preparations that our patients might use include:
Tri-caine topical gel which consists
of 20% benzocaine, 6% lidocaine, and 4% tetracaine is a great method to numb
the area before doing a simple dental procedure or before injecting the much
dreaded “SHOT”.
Topical NSAIDS can be used for
painful ulceration, oral and perioral pain (neuropathies or neuralgias), and
sore muscles of the jaw due to long duration of dental procedures. Also, Transdermal
NSAIDS and cyclobenzapine can be used to relieve pain resulting from temporal-mandibular joint disorder (TMJ). This formulation can also be applied
to the aching neck and hands of dentists and dental technicians. Studies have
shown that transdermal NSAIDS have a significantly higher tissue level of the
medication when applied to the area topically than through the traditional oral
dosage formulations. Furthermore, topical NSAID formulations can also circumvent
the uncomfortable side effects of NSAIDS such as gastrointestinal irritation. In addition to this, topical NSAIDS are advantageous to patients because they can be less costly than other pain medication alternatives.
The topical/transdermal preparations that we can prepare for our patients so that the pain and spasms associated with their TMD/TMJ can be controlled includes:
Ketoprofen 10%, Magnesium 5% gel
Ketoprofen 10%, Cyclobenzaprine 1% PLO
Diclofenac 5%, Magnesium 5% gel
Depending on the type of pain, a combination of an NSAID with an anesthetic such as lidocaine or tetracaine, and/or gabapentin or carbamazapine for nerve pain can be formulated.
Unique Oral Delivery
When patients have problems taking
medications, we have the solution. How about a lollipop that contains the
medication you need and tastes great. In addition to lollipops, we can also
make frozen popsicles, chewable gummy treats, solutions, troches, or even
medicated toothpaste. These formulations are great for children and geriatric
patients. We also customize medications for diabetic patients (sugar-free), chemotherapy patients (alcohol-free), and
gastrointestinal irritation (lactose-free) for patients with gastrointestinal irritations. These formulations and delivery systems can greatly increase patient compliance and procedure outcomes. Some of these products include:
Xylitol gummy treats are great for
the oral health of any patient because it decreases plaque formation by
inhibiting bacteria’s ability to stick to teeth.
Topical antibiotics or anti-fungal
formulated into flavored lollipops are an excellent way to ensure medication
absorption in the oral cavity.
Anti-gag troches can also be
formulated to suppress the gag reflex. These contain a variety of electrolytes
and are retained intraorally a few minutes before the start of the procedure.
Chlorhexadine can be formulated into
a gel or toothpaste for patients with periodontal disease. These formulations
are useful when patients are not able to swish and spit.
Tranexamic acid solution can also be
prepared as mouthwash to prevent post oral surgery bleeding without dose
modification of oral anticoagulants.
